Steps to become a client:

We accept most public and private insurance plans.

We also offer special pricing for cash-paying patients for most services.

Private Pay Client

 

  • Contact Clarke Community Care to discuss the need for potential services

  • Clarke Community Care nurses will assess the client to formulate Plan of Care

  • Finalize contract for services to be provided and rate

Community Alternative Program (CAP) Client

 

  • The client should contact their Case Manager to request services from Clarke Community Care.

  • Case Manager sends authorization for services to Clarke Community Care services.

  • Upon completion of the assessment, the Clarke Community Care nurse will assess the client to formulate Plan of Care.

 

Medicaid Recipient Client

 

  • The client should take a referral request form to a Primary Care Provider (PCP) for completion.

  • Liberty nurses will contact the client for nurse assessment.

  • Upon completion of the assessment, the Clarke Community Care nurse will assess the client to formulate Plan of Care.

  • Have PCP to fax referral request to Liberty for personal care services.

Veteran Client

 

  • Contact Your Primary Veteran Physician for Program Qualifications.

  • Contact Clarke Community Care to discuss the need for potential services.

  • Upon completion of the assessment the Clarke Community Care nurse will assess the client to formulate Plan of Care.
