Steps to become a client:
We accept most public and private insurance plans.
We also offer special pricing for cash-paying patients for most services.
Private Pay Client

Contact Clarke Community Care to discuss the need for potential services
-
Clarke Community Care nurses will assess the client to formulate Plan of Care
-
Finalize contract for services to be provided and rate
Community Alternative Program (CAP) Client
-
The client should contact their Case Manager to request services from Clarke Community Care.

Case Manager sends authorization for services to Clarke Community Care services.

Upon completion of the assessment, the Clarke Community Care nurse will assess the client to formulate Plan of Care.
Medicaid Recipient Client

The client should take a referral request form to a Primary Care Provider (PCP) for completion.

Liberty nurses will contact the client for nurse assessment.

Upon completion of the assessment, the Clarke Community Care nurse will assess the client to formulate Plan of Care.

Have PCP to fax referral request to Liberty for personal care services.
Veteran Client

Contact Your Primary Veteran Physician for Program Qualifications.

Contact Clarke Community Care to discuss the need for potential services.
